an acute attack in the left first metatarso-phalangeal joint. Since then, he has had attacks affecting most of the joints of the lower and upper limbs, especially the metatarsophalangeal joints of the big toes. At the time of examination he had swelling of the first metatarso-phalangeal joint and ankle on the left side, and of the right metacarpophalangeal joint. He had very extensive tophi scattered about t'he fasciae and tendons of the dorsum of the right foot and of the left foot also. He had very big tophi over both elbow joints and over the head of the right fibula, and a few scattered smaller ones in the aseia lata of the right thigh. He frequently gets tophi over the knuckles.
